Are colonoscopies actually helping (women and society)? Colorectal cancer (the second leading cause of cancer deaths in the U.S.) is one of the most preventable because it usually develops predictably over about 10 years and colonoscopy can both find and remove polyps in the same visit. She contrasts this with mammograms (episode 258) and explains why colonoscopy payoff is high but not perfect: prep quality and provider skill strongly affect what gets missed, with an adenoma miss rate around 24%. She reviews updated screening guidance to start at 45 (earlier with risk factors), discusses alternative tests like Cologuard and SHIELD, and weighs real risks—prep complications, perforation, bleeding, and sedation—against the dramatic survival difference between localized and metastatic disease.
